If you area  new patient to Beach Cities Orthopedics & Sports Medicine, please fill out a New Patient Intake form (regular or minor), New Patient Medical History Form (regular or minor), Welcome Letter, Communication Form, Patient Financial Responsibility Form, and Opioid Prescription Contract. If you have a specific injury to the spine, shoulder, or knee, please fill out the corresponding form as well and bring your completed forms to your first appointment.
